Covenant Community Care Family Dental Center will be providing some free professional dental preventive services at Stout Middle School on a later date. The services include dental examinations, x-rays, teeth cleaning, sealants, and fluorides. There will be no out of pocket cost for preventive services performed at your child’s school. The deadline is Friday, Jan. 8, 2016.
- Parents must visit the school blog;
- Download and complete the medical history and permission form;
- Return it to the school office.
You can also get the form from the main office or from Mr. Sabra’s office to get the form.
