HAP members – Present HAP Card only to the nurse.
All others – a front & back copy of insurance card and Driver’s License.
Insurance Plans accepted are BCN, BCBSM, Aetna, United Health and Priority Health.
Please make check payable directly to Henry Ford Health System. Please remember to wear clothing which allows for easy access to your upper arm.
