dentists-r-usA reminder to take advantage of this opportunity to get your child some free preventive dental services to help maintain his / her teeth health, specially if you do not have dental coverage . These free services will be performed on Friday, Dec. 16, 2016 at Stout Middle School. All you have to do is:

  1.         Click on the link below.
  2.         Print the 4rth page.
  3.         Fill it out.
  4.         Send it with your child to Stout Middle School.

Ask your child to visit the Community Liaison, Mr. Sabra, who will print the form for him / her. You can also pick up the form during the Parent-Teacher Conferences on Monday, Oct. 24th or Wednesday, Oct. 26th between 4:00 pm and 7:00 pm.
