All students planning to participate in a sport at Stout Middle School during the 2024-25 school year must have a “sports physical” completed and turned into the Stout office before participating in the sport. 

We are excited to announce that Henry Ford Health Care will be at Stout Middle School on Wednesday, September 4, 2024 from 8:00 AM-12:00 PM to offer  a “FREE” exam for students who still need their sports physical completed. 

All parents need to do is  fill out both form(s) found in the main office and return them to Stout with your child by the morning of Wednesday, September 4 to allow permission for students to be seen by the Henry Ford Health Care Provider. Students are welcome to pick up the forms to take home to be filled out by the parent and return back to school.
